Updating us on your list of medications and medical providers allows us to shop around to find big savings, and to ensure your medications and providers are covered by your insurance in 2025.

This form allows me to discuss your plan updates for 2025. It does not sign you up for anything, and does not obligate you to anything.  You have signed this form before, we just need an updated copy.
